What's the right strategy for managing context windows in long document workflows?

We're processing legal contracts that regularly exceed 100k tokens. We've tried naive chunking, sliding windows, and hierarchical summarization. Each approach loses something important. How do practitioners decide what to keep in context vs. what to retrieve, especially when the model needs to reason across distant sections of the same document?
