Checking on the patient the day after surgery

A patient in trouble is heard the next morning rather than at night on the emergency line, and the practice learns about it as

The Switchboard, seen from above on the Cabinet floor plan.
Exhibit 1. The Switchboard, at its place in the room.

A patient goes home after an extraction or a root canal with post-operative instructions and no follow-up unless somebody at the desk remembers to ring. The ones who are struggling either ring the emergency line at night or wait, and the practice hears about it as a complaint rather than as a symptom. If you own a dental group, that is a desk in your own business.

What the work is

An agent that opens a short conversation with every surgical patient the next day, asks about pain, bleeding and swelling in plain terms, restates the post-operative instructions, and routes any answer that suggests a problem straight to the on-call clinician with the chart open. In the Cabinet, ArkOne’s reference design for an executive agent, this is the Triage seat: one of ten specialists the room can consult, each a name, a model and a set of instructions rather than a department.

The part of the design carrying the weight here is the switchboard, and the setting that governs it is stated once on the Register.

What it does not do

Any clinical response to a reported symptom, including whether the patient is seen today, is made by a clinician; the agent never advises on medication or reassures a patient who reports pain. That is not a limitation of the software. It is the line the reference design draws on purpose, and a vendor who offers to move it is selling you something else.

What changes

A patient in trouble is heard the next morning rather than at night on the emergency line, and the practice learns about it as a symptom rather than as a complaint.

Take post-operative instructions, surgical extraction, dry socket into the demonstration and use them without explaining them. A vendor whose agent handles this work already knows those words; one who asks what they mean is showing you a system built for a different trade.

Assembly · the Seats

The Seat that owns this work gathers what it needs and drafts. This is the part a demonstration shows you, and it is the part that cannot break anything.

The hours are a shape, not a schedule. What is fixed is the order: nothing reaches a system you run before the Door, and nothing continues past the wait without a person.
